as far as i can tell, Barcelona's traffic engineers take a layered approach. the first layer is that most of the streets are one-way. this immediately eliminates a particularly dangerous form of traffic (left turns across oncoming traffic). it also means that they can put the crosswalk on the "oncoming traffic side" of intersections, so all turning vehicles are turning **away** from where the pedestrians and cyclists are crossing.
